It collects names, addresses, dates of birth, email addresses, telephone numbers and background information, in order to communicate with clients and provide the services offered, planning dyslexia assessments and/or specialist lessons.
How Does Talk Dyslexia Channel Islands Collect Data?
Personal data includes information which originates from questionnaires completed by clients, employers, parents, pupils, or schools, such as full name, address, date of birth, email address and contact information. It also includes information received during telephone calls, interviews, written and verbal correspondence.
How Does Talk Dyslexia Channel Islands Store Data?
Assessment reports are stored electronically on a password-protected computer. Paper record forms are destroyed once the final report is issued. Due to the confidentiality, security, and integrity of the assessment materials, copies of assessment materials or score sheets are not allowed to be shared. Where applicable, scanned copies of record forms are stored as PDF documents on a password-protected computer. Reports for children under 18 will be kept until their 25th birthday; for adults over 18, reports will be kept for 7 years.
Does Talk Dyslexia Channel Islands Share Data?
Talk Dyslexia Channel Islands will only share data when there is a lawful basis to do so. Reports will not be shared with any third party (e.g. schools, colleges, universities, workplace) without client consent.
